★ ★ ★ ★ ★ The department provides medical report for all sort purpose such as Normal Medical Report, Post Mortem Report, insurance Claim Report, EPF Withdrawal Form, SOCSO Form, Claim for Compensation by lawyer, Employer Reference, Insurance Claim, EPF Withdrawal and SOCSO Claim.
You need to fill up Consent Application for Medical Report form and attach a copy of your NRIC and Appointment Card. There is a convenient store just beside the department, just in case you need to make a photo copy of your documents.
Of course you will be charged to obtain the report plus GST.
The requested medical report is not ready on the same day of your submission. They will inform you to collect when it is ready. Please make sure to write phone number in the form.
